HCV Application Process
LMH opens its HCV waiting list on an as needed basis. When LMH determines that the list is to be opened, public notice of the opening will be made in the local newspaper and through other media as determined necessary to reach potentially eligible households not otherwise likely to apply.
The public notice will state that pre-applications for the Housing Choice Voucher Program will be accepted, how they will be accepted and the time frame that the waiting list will be open. Since the demand for housing assistance often exceeds the limited resources available to HUD and the local public housing agencies, long waiting periods are common and therefore LMH uses a two-step application process. Under the two-step application process, an applicant will complete a pre-application for purposes of placement onto the waiting list only (step one). Step two is when the applicant is actually selected from the waiting list and invited to complete a full application where they will provide all necessary information needed to establish eligibility.
At the conclusion of the pre-application period, a lottery system will be used to determine the applicants who will be placed on the waiting list. These applicants will be randomly assigned a number to their pre-application. Applicants are placed on the waiting list in order of the assigned numbers and according to LMH preference(s).
Applicants are selected from the waiting list and invited to complete a full application in accordance first with LMH's preferences and then in order of the randomly assigned pre-application number.
Full applications are taken in small groups and by invitation only at LMH's Byrne Road office located at 211 S Byrne.
Completing a Full Application
Applicants need to bring the following at the time of application:
- Birth certificates for all persons to be on application
- Social Security cards for all persons on application
- Picture ID for all persons over the age of 18 on the application
- All adults on application are required to sign all forms
- Citizenship / Alien status for persons born outside U.S.
- Verification of assets of all household members
- Verification of income for all household members
- Examples of income included but are not limited to:
- Social Security
- Child support
- Retirement or disability pension benefits
- VA benefits
- Bank interest
- Any other source of income
If you do not provide all the information at the time of your application, it will cause a delay in processing.
If you received a letter that your name has reached the top of the Housing Choice Voucher Program's waiting list and have been invited to complete a full application, please access the application below. Return the completed application to our office by the deadline date indicated in your letter.
