CERTIFICATION
I certify that the information on this application is true and accurate.  I understand and agree that LMHA will use this information to pre-qualify me so that I may be considered an LMHA qualified contractor/vendor. I agree that I will pay the appropriate HUD wages or Davis-Bacon wages. I agree to abide by LMHA’s Statement of Procurement Policy and applicable HUD regulations, both of which may be amended from time to time. I understand and agree that this information is confidential and will be used for no other purpose.  I understand that this application, if approved, will remain in full force and effect until the end of the calendar year. I understand and agree that I will need to qualify every January. I understand and agree that LMHA reserves the right to demand supplemental information. I agree that I will inform LMHA of any material changes within ten (10) days of occurrence. I understand that LMHA may revoke my status for false or deceptive statements or for other cause and that I may file a protest of such revocation in accordance with Section VII, Appeals and Remedies in LMHA’s Statement of Procurement.  I understand that if I lose my appeal, LMHA may deny my participation in LMHA’s procurement processes for a period of six (6) months from the date of such determination.
